To participate, please provide the following information on your company's letterhead. If you would like to place an order as soon as your account is set up, please note that on the letterhead and we will call you to take your order once your account is active.
On your company's letterhead, please include:
- Company Name
- Address
- Contact Name(s) (allowed up to 3)
- Phone number(s)
- Email address(es)
Along with your letterhead, please include a copy of your business' W-9 tax identification form. Send your documents to us in one of three ways:
- Fax both documents to 717-960-1356 Attn: Gift Card Department
- Email both documents as a pdf to ausa.giftcard.group@ahold.com
- Mail both documents to:

New account set-up takes 7 to 10 days to process. You will receive a letter in the mail, which will include your account number and ordering instructions. Orders take 5 to 7 business days from the date of order to arrive at the requested store location.
